Image missing in submitted workflow (quick start app)

After submitting a workflow with iPhone, the images in the submitted workflow (Manager’s view) are often not accessible (30-40%?). Can you let me know how to make sure images with workflow are properly submitted?

Can you confirm that you’re using Skylight iOS 3.1.2? We have run into inconsistencies on previous builds but are seeing expected behavior on the latest (3.1.2).
